Ans)1) The Patient-Centered Medical Home (PCMH) is a care delivery model whereby patient treatment is coordinated through their primary care physician to ensure they receive the necessary care when and where they need it, in a manner they can understand.

Around five core principles and functions:

1) comprehensive care

2) a patient-centered approach

3) coordinated care

4) accessibility of services

5) quality and safety of care, the PCMH is committed to quality improvement (QI), performance improvement, patient satisfaction, and population health management.

2) The purpose of this study was to initiate a collective inquiry regarding safety net
services in King County and explore how to increase effective and efficient access to
care for the population utilizing the system.

- The component conducted by MCPP
included interviewing key stakeholders currently engaged in the provision of safety
net services.
- Interviews and content analyses were conducted by MCPP, with strong participation and support from Public Health - Seattle & King County and key provider and non-provider stakeholders of the core health safety net in King County.
