1. Altered mentally status revealed to disease condition.

Intervention with rational:

1. Assess patient level of consciousness and change in behaviors.

Rational:

It provide baseline comparison with ongoing assessment finding.

2. Give well ventilated room and side rails up.

Rational:

To prevent fall injury.

3. Limit noise and environment stimuli

Rational:

To prevent additional confusion

4. Speak slowly and clearly.

Rational

Reduce frustration.

Intervention:

1. Assessed patient level of consciousness.

2. Well ventilated room and bed rails are padded up.

3. Environment noise and stimuli are reduced.

4. Communication maintained with slowly with soft voice.

II. Self care deficit related to hygienic activities

Intervention with rational:

1. Assess the client abilities to bath self through direct assessment.

Rational:

Thus helps to provide comprehensive assessment data about the patients.

2. Plan the activities that avoid fatigue during bath.

Rational:

Energy conservation increases the activity tolerance and prevent fatigue.

3. Use consistent routine and adequate time to complete task.

Rational:

Help the patient to compare and carryout self care activities.

4. Provide privacy during bathing and dressing.

Rational:

Need for privacy is fundamental for the patients

Intervention:

1.through assessment made on self care abilities of the patients.

2. Bath is planned early in the morning with full energy of patient.

3. Routine environment and adequate time provided to patient to complete the task.

4. Privacy is provided during bathing and dressing.
