Possible forms of treatment are outlined, including surgery, chemotherapy, and/or radiation, and a careful explanation of the risks and possible outcomes of each are outlined.Sometimes a provider encourages a particular form of treatment even before the client asks for a recommendation. Here the oncologist has knowledge of Phil's particular case and may be expected to explain the possible risks of no treatment at all. Although the client often looks to the specialist for medical information and recommendations, the decision is the client's alone. 3. Reasonable alternatives

4. Risk and benefits of the alternatives

5. Assessment of the patients understanding from the element 1 through 4

So here in this Phil's nature, risk and benefits of the procedure has been explained by the provider but the other three components possible alternatives , it's risk and benefits as well as assessment of patients understanding of the procedure has not been covered.
