Answer: The cause of the high blood pressure, edema, and rhonchi are as patient is having congestive heart failure. In this diseased condition, the heart of the patient becomes unable to pump normal amount of the blood, hence this causes low supply of blood through the tissues. This causes increase in the blood pressure whereas, insufficient supply of oxygen causes rhonchi. Patient is not taking medicine so these symptoms are increased.

The nurse should intervene about the proper medication intake by the patient as well as the drugs that has been prescribed are regularly consumed by the patient. The nurse should educate the patient regarding medicine intake. The should intervene about the presence of productive cough because it could be the sign that patient may have lung infections such as pneumonia etc.

To become culturally competence, nurse should indulge some steps such as:
