An Adult Gerontology Primary Care Nurse Practitioner (AGPCNP) is a type of a nurse practitioner who specialises in caring for patients from adolescence to adulthood to old age. AGPCNPs provide acute , chronic and preventive health care services, coordinating with speciality physicians and others health care providers as needed. They offer routine checkups, assessment, immunisations, health counselling and education, diagnosing, examining and treating their patients. They work with patients and their family members to develop and implement healthy lifestyle and disease prevention plans. They may have their own private practice or may also work in clinics , hospitals, rehabilitation centres or nursing homes.
