Long term care - providing health care services to an individual for more than 90 days

Facilities of longterm care include

-Hospitals/ Nursing homes

-Assisted living/ community based residential facilities

-Adult day care/ Adult foster home

-Respite care/ Hospice care

-Rehabilitation centers/ Mental

-Children's chronic care facilities/ Care provided by family and friends

Continuum of care :-

Concept involving and integrated system of care that guides and tracks patients over time through a comprehensive array of health services spanning all levels of intensity of care

-Long-term care: A broad range of services including medical, social, and residential care provided over a long period of time to people with chronic disease of disability.

-Level of services required; early, moderate, or advanced levels. A patient can enter the continuum at any level.

Hospice care and it's perspectives :-

Family-centered care that allows patients to live and remain at home; focuses on palliative (not curative) care: comfort, independence, and dignity; provides patient and family support during terminal illness and time of death; many hospice programs provide respite care, which is important in maintaining the health of the primary caregiver and family.
