7.D. Past Health History.

Genogram is a pictoral representation of a person's familial relationships and medical history.

Family history cannot be represented as a genogram but familial relationships can be.Also Chief complaints and current health status cannot be represented.

8.C. "Is there anything else you would like to mention".

The interview should be closed at a right note by giving a lasting impression. So all other options should be omitted. Asking "Did we forget something'' leaves an impression that the interviewer is not prepared.

"I need to go on to the next patient.I will be back" statement shows that the interviewer is leaving to the next patient without concluding the interview.

"While i am here let's talk about your upcoming surgery" statement is not appropriate for concluding an interview.

9. A. Organize assessment to minimize changes in patient position.

It will reduce the discomfort of the patient.

If we examine painful areas first,patient will not allow the nurse to do further examination as it can alleviate pain.

Examination should be performed from the right side of the client and not from the left side.

Same examination sequence cannot be performed for all the clients as the patients have different diagnosis.

10. C. 12.5 pack years.

Number of cigarettes in a pack = 20 cigarettes.

1/2 pack cigarette means 10 cigarettes.

Therefore, Number of pack years = number of cigarettes smoked per day * number of years smoked / 20

= 10 * 5 / 20

= 50 / 20 = 2.5 pack years.

He stopped cigarette for 2 years. Again started smoking 1 pack (20 cigarettes) per day.

Therefore, Number of pack years = 20 * 10 / 20

= 200 / 20 = 10 pack years.

So,Total pack years =2.5 + 10 = 12.5 pack years.

11. D. " This will be documented and you will not receive any penicillin."

This statement gives the patient an assurance that the nurse have taken the matter into consideration and they will not receive any penicillin.Other Statements does not do so.

12. A,B, C,D,E

If language is not understandable it will affect the communication.If the patient is uneducated he can't communicate properly.

Sensory impairments like hearing loss affects communication.

Physical environment like noise also affects communication.

Lack of privacy also affects communication as the patient will not be able to speak out important matters which he don't want others to listen.
