HOMEWORK Ch. 15-Integumentary (Skin) 1. 10 year old Ricky was admitted because of severe cellulitis of the left leg. He had gone on a hiking trip in the nearby forest with his Boy Scout troop a week earlier and now has a painful reddened area on the left leg. He stated that there was a good deal of thorny brush and that he had several minor thorny brush and that he had experienced some minor thorny punctures of the left leg but they didn't bother him. The day before admission, he had developed a painful swollen area that had become worse during the night. A diagnosis of cellulitis due to Streptococcus A was made, and antibiotics were administered intravenously. The wound was evaluated but did not appear to need specific treatment. The area on the leg progressively healed. He was discharged to continue the oral antibiotic series at home and to visit his pediatrician in one week. Code this case. 2. An 89-year-old female patient was admitted from the nursing home due to concern that her pressure ulcer of the buttock is worsening. Upon admission it was noted by the attending physician that the patient has a stage lli pressure ulcer to the left buttock. Documentation by the attending physician in the history and physical examination stated the patient also had a stage ll pressure ulcer of the left ankle. Per the physician admission orders, the patient was followed by the wound care nurse for treatment of her ulcers. A surgical consultation was also requested during the hospital stay. The surgeon recommended debridement of these ulcers. At the patient bedside the surgeon performed an excisional debridement of skin and subcutaneous tissue of the buttock ulcer and an excisional debridement of skin of the ankle ulcer. The patient was discharged back to the nursing home for continued care. Code this case.
