4) C) Plan.

A SOAP note is a kind of note which carries a patient's information. The note is being presented in a specific order. There are four sections of a SOAP note. The plan is the last section where the practitioners seek ways to address the patient's problem. It includes medication, taking a few days off to rest completely, the recommendation for time to time appointments, etc.

7) B) Subjective.

This is the first section of a SOAP note. In this section, a patient or his or her caregiver presents the complaints for which he or she came. The patient verbally expresses the symptoms which also includes past symptoms. The practitioners then diagnose based on the past and current information regarding the symptoms.
