\rightarrow mixing of two drugs I.e. drug drug incompatibility

\rightarrow drug and drug container reaction.

  • The unintended presence of precipitation and toxic products can cause various negative consequences for the patient.

\rightarrowtissue irritation due to major ph Changes

\rightarrow emboli formation due to crystallisation of drug.

\rightarrowtherapeutic failure.

  • Nitrates and chlorides are highly soluble and have few precipitates among metal salts.
  • However, sulfates form a low solubility salt with calcium, common in many medications.
  • And phosphate also forms low solubility salts with calcium and magnesium that are in medications.
