Answer #1

Ans: There are two factors whichindluence the process while collecting the data are, primary and secondary data:

Primary data: It always includes relevant situation which is currently happening and verifying all the facts, it follows a specific set of process and questionnaire, primary data always helps in framing the facts in a disciplainary manner.

Secondary data: it always includes the information which has been already collected by someone and you are using it as a refrence to take further steps in certain situations.

The following are the different possibilities to stratify the data for better analysis:

1. Adequate knowledge about clinical characteristics and previous treatment of individual patients.

2. Through error-reporting systems we can prevent scribing faults and prescription errors.

3. In prescribing process, monitoring of drug action is mandatory and need to have a track check of doses or treatments.

4. Need to have a knowledge in pharmacotherapy to Transcrib previous medications and collecting information of medication history about the patient's clinical condition.

5. Maintain a Specific Questionnaire to avoid conflicts
