Community health is a field of public health centered on the study and enhancement of health characteristics of biological communities. It tend to focus services, education and research on geographical areas with shared characteristics. Community health is often seen as a field of within public health focusing on the health of a particular population group that has common charactetistics such as culture, work, traits geography or other demographics.
Population health is concerned with distribution of health outcome across a group of individuals. It includes health outcome ,pattern of health determinants and policies and intervention that link the two. Population health aims at improving the quality of care, improving health of population and reducing per capita cost of health care. Its a health improvement strategy for risk based entities, such as managed care plans, self insured employers, and accountable care organisation that are financially responsible for clinical and economical outcome of beneficiaries under their care.
