The client is actively hallucinating and having paranoid delusions The client states that people are watching him and trying to kill him. You notice the client engaging in conversations without anyone nearby. The client is dirty with unkempt hair and clothes. The client is thin and malnourished appearing. The client has dry oral mucosa and halitosis. The client states that he is homeless, has no family or friends and has been out of his medications for one month. The client's speech is sometimes incoherent and he rapidly changes subjects with little association. Upon physical assessment, you find the following: Normal findings of heart, lungs and abdomen. Mucosa are dry and skin turgor is poor. Client is markedly underweight and hygiene is poor. You note the presence of body lice. Vital signs are: P88, Temp 98,7, Resp 20, B/P 100/64 Drug testing is positive for alcohol and marijuana. He graduated from high school and started attending college but had to drop out in his first year due to symptoms of schizophrenia appearing He has been arrested several times for minor things and currently has a pending court date for trespassing. He is currently homeless. He was originally living with his family, but they could no longer handle the situation and so they found a group home for him. He left the group home about 6 months ago and has been homeless since that time. The family is minimally involved. The parents and siblings of the client have never been diagnosed with a mental illness. The client has an uncle that has been in a state mental hospital for years", but the diagnosis is not known to the patient.
