1. Toyota is not the cause for the unintended acceleration. However, it is blamed for it since Toyota has been a complete game changer in the automobile industry. It's own Toyota Production System paved way for quality improvement system and improved productivity system which we call today as Lean Manufacturing. The focus has shifted from manufacturers to customers by the system formed by Toyota.
2. The reason for this is that all automobile manufacturers eagerly begin to adopt lean management concept, tools and techniques. However, they fail to understand that, lean is a gradual process and require time and effort along with patience and persistence to improve processes and ultimately increase productivity and quality, which results in increase in customer satisfaction and growth of the organization.
3. Yes. It is possible. Self-discipline and sincerity are two of the main attitudes the organizational culture must possess. Also, the employees and the organization must be customer-centric, such that all are willing to go an extra mile in order to satisfy customers. It is the attitude to improve performance and perfect self and processes which leads to higher productivity.
4. I would not be surprised, as I would know that Toyota is the first to adopt the Lean manufacturing concept as it's roots are in it's own developed Toyota Production System. However, would realize that this is just a big step for the company, and improvements are ongoing and mustn't be interrupted by such successes.
