5.) The ongoing upper principal ray converges from its initial path and also ray gets refracted through optical device which means its a lens not a mirror. Thus the optical device is converging lens i.e. convex lens.
option-iii is correct

6.) The ongoing upper principal ray diverges from its initial path which indicates that optical device is diverging one. Also the ray gets transmitted through the optical device which shows that the optical device is not a mirror. Hence we have a diverging lens as the optical device i.e. Concave lens.
