Nursing interventions and rationale

• Monitor the vital signs,  baseline data to plan for care
• Administer anticholenergic drugs to control the incontinence
• Teach Kegel exercise,to strengthen pelvic floor muscles
• Teach perineal hygiene to prevent infection
• Provide psychological support to reduce anxiety and anger in social activities
• Avoiding tea and coffee as they are stimulants

Evaluation

The patient verbalises she is able to control urine and reduced urgency to void.
