Option 2 is true, since fibrillations can occur in ventricles. This is called ventricular fibrillation and is caused due to disordered electrical activity in the ventricles, causing them to contract irregularly and rapidly.

Option 3 is true. Certain injuries in the Achilles tendon, such as Achilles tendinosis, are associated with fibrillation of the Achilles tendon.

So, only two of the above are true regarding fibrillations. Therefore, option 4 is the correct answer.
