Probability of an item being defective = 0.1

So, probability that an item is not defective = 0.9

So, probability that the first defective item found is the fifth item inspected =

= Probability that the first 4 items inspected is not defective * Probability that the fifth item is defective

= 0.9*0.9*0.9*0.9*0.1

= 0.06561.
