There are 2 pictures but the final part should be one

Model the part using SolidWorks' sheetmetal tools. Use HSMWorks to program the machining on the part; face the top surfaces and drill/tap two holes. More points will be given the more realistic the simulation (ie if you create a "bent" configuration and use it as the stock for the machining) Tooling is up to you, place the coordinate system origin somewhere logical for touching off, make the stock as realistic as you can.

Use Base Flange/Tab feature

Use Edge flange feature and select an edge:

Do this twice for both sides.

enter 0.75'' as length

Now start making holes using hole wizard.

Select the options shown below. Then go to positions tab

Select the plane on the top of the bent flange and make the following sketch.

Then press OK. Your part is ready.
