Therefore, external forces may relate to multiple external factors including economic, social, technological, or political factors and dimensions. Internal forces of change may also necessitate change and adaptation. A decline in effectiveness, deficiencies of existing systems, crisis employees and their expectations, and change in managerial personnel and/or work climate act as an internal force for a change to happen.

Because Antonia wants the hospital to purchase a new piece of expensive equipment, the force that makes her a change leader here is a technological force. Technological innovations are important for competitiveness and may also improve the quality or the efficiency of services and products including medical services. New medical devices or machines may help in better diagnosis, treatment, and care management among other benefits. Technological change may also provide for decentralized decision making, flatter structures, and a more open communication model. Technological forces of change are even more relevant to the medical practitioners (including surgeons) and healthcare facilities/organizations, as they are frequent, improve the accuracy and quality of services, provide for qualitative improvements, and may also be vital for the patients.
