1) Calculation of volume(ml) of acetaminophen required.

Formula

Volume(ml) = Desired dose(D)/Dose in hand(H) * Quantity(Q).

Here

Ordered dose = 10 mg/kg.

Weight of the child = 10 lb.

Convert lb to kg for calculation

1 lb = 0.454 kg.

Weight of this patient = 10 * 0.454

= 4.5 kg.

Dose required for this patient = 10 * 4.5

= 45 mg.

Dose in hand(H) = 15 mg.

Quantity(Q) = 5 ml.

As per above formula

Volume(ml) of drug required = D/H * Q

= 45/15 * 5

= 3 * 5

= 15 ml.

2) c) Safe 24hr range in mg.

Ordered dose = 100 mg every 6h.

Safe dose range = 25 mg/kg/24hr - 30 mg/kg/24hr.

Weight of the patient = 15 lbs.

Convert lbs to kg for calculation

1 lbs = 0.454 kg.

Weight of the patient = 15 * 0.454

= 6.8 kg.

Dose(mg) range for this patient = 25 * 6.8 to 30 * 6.8

= 170 mg to 204 mg per 24 hr.

d) Ordered dose is safe or not.
