If you look at the complete C4v character table (Not given in this question), you will see that symmetry of 5 d-orbitals can be evaluated from column corresponding to quadratic functions which corresponds to the d-orbitals only. Otherwise, you can determine the same by applying every operation on each d-orbital and observe the changes made, then co-relate it to the character table.
