1 vial of Injection CARBENICILLIN contain 5 gram = 5000 mg
Add 9.5 ml sterile water in above injection for a final concentration of 500 mg/ ml
So 5000 mg (1 vial) = 10 ml solution after dilution
that means 10ml - 9.5 ml sterile water = 0.5 ml of IV CARBENICILLIN concentrated solution contain 5 gram.
Now physician order : 750 mg IV hourly
1 dose = 750 mg of injection CARBENICILLIN
Coz final concentration of injection CARBENICILLIN contain 500 mg = 1 ml
Therefore, 750 mg = 1/500 X 750 ml = 1.5 ml
1.5 ml solution are required per dose.
Most of IV injection preparation commercially available in the active drug form either solution or suspension. Here we add 9.5 ml of sterile water to final concentration 500mg/ ml that means active drug form in liquid solution of 0.5 ml per vial. That's why concentration 500mg/ ml instead of 526.3 mg/ml.

Here injection KEFZOL 1 vial = 1000 mg
Adding 2.5 ml of sterile water to final concentration of 330 mg / ml
330 mg of injection KEFZOL = 1 ml solution
So 1000 mg of injection KEFZOL ( 1 vial) = 1/ 330x 100 = 3 ml total solution
That means 0.5 ml of injection KEFZOL of active drug in the form of solution
Now physician order : 165 mg IV 8 hourly
Here 330 mg = 1 ml solution of injection KEFZOL
So , 165 mg = 1/340x 165 = 0.5 ml of final concentration of injection KEFZOL
Therefore, 0.5 ml solution of injection KEFZOL required per dose.
