2) I have used FMECA safety values to rate the causes.

3) I have written severity ranking in front of the cause itself.

Solution:

Accident anaysis:

There are some steps to be followed for accident analysis. Firstly, i would like to state causes here.

Causes:

1) The previous 4 incidents were not reported. 10

2) Corrective action was not taken on the very first accident. 10

3) Faulty drainage lines . 8

4) Slippery floor and cheap plastic mats. 8

5) Used footwear. 2

6) If this has happened at most of the franchise then the management mst have used cheapo materials for tiles and mats to save money. 10

Conclusion:

It can be concluded from the causes that the management must be hold responsible for the installation of cheap equipments.

Injury:

The injury to the kitchen worker is severe. An injury to skull or a skull fracture indicates the severity of accident.

The injury leads to following effects to Hotel Management:

1) The person may go into coma 8

2) The person may die 8

3) The person may lose hearing and memory 8

4) Relatives of person sue hotel and the previous accidents will only strengthen the case in favor of kitchen workers. 8

Conclusion:

There are high chances that the workers will revolt and the person caught in the acccident will suffer physical as well as mental trauma.

Suggested Action:

The suggested actions are as follows:

1) Change the flooring to rough tiles

2) Provide financial aid to families of injured.

3) Find professional agencies to help with problem.

4) Get the drainage lined checked.

5) Invest in insurance.

Conclusion:

If the owner is ready then the management of hotel should be changed and also the hotel must be refurbished to prevent further damages.
