It is not unusual for maintenance or updates to an engineered system to result in substantial alterations to the original design. Can you think of other safety critical engineered systems where retrofits might alter the safety of a system and contribute to failures?

Every engineering systems are designed after a lot of research,development and testing.Altering it's initial structure for retrofit without proper knowledge may lead to the failure of the system.The best examples is from our day to day life

Vechicles

The alternation of engine power for better performance or better economy(mileage) without considering other facts leads to shortening it's working period.Suppose i make alternations to the engine to get more power and I became successful.But then the working of important parts such as pistons,crankshaft etc are increased ,the temperature inside the engine are also increases but the engine is not constructed to withstand the conditions above it's standard operating conditions .As a result eventually it wears off and result in failure
