Pivot table is created using Insert Pivot table feature in Excel as below:

Calculated Fields are added by clicking on Calcuated Fields option in "Fields, Items"

Following formulas are used to create the Calculated Fields

Field Formula
$/Visit =Spend/Visits
$/Direct Signup =Spend/'Customers(D)'
$/Survey Signup =Spend/'Customers(s)'
Visit to Direct Signup CVR =Visits/'Customers(D)'

Resulting Pivot table is following:
