1. Old, Upatched Security vulnerabilities

2. Human Error such as weak passwords, sending sensitive information to wrong recepients , sharing passwords / account information, failing for phishing scams

3. Malware - A type of virus caused due to weak firewall at systems

4. Insider Misuse - Intentional misuse of data by authorized personnel for personal gain

5. Physical Theft of a data carrying device such as external hard disk, pen drive, CD or DVD, Mobile, laptop or even servers.
