Why do the public and private healthcare sectors have a lack of alignment or sharing of quality initiatives and data, particularly the quality reporting for hospitals required by CMS? What are possible solutions that could be implemented to benefit the public? How could health information technology (HIT) help link the public and private sectors with quality initiatives? Since Dr. McClellan's testimony, do the current VBC (value-based care) initiatives support the alignment or sharing of quality initiatives that would benefit the population's health? 1. Poor quality initiatives and data due to lack of alignment or sharing is dependent on the health care system in a country.every individual country has their own quality guidelines laid by their governing bodies.lack of alignment or sharing of quality initiative and data in public and private healthcare sectors is directly dependent on the development status of their country. A poor outcome is because of the health care system in a country with low or middle income output.Private sector healthcare delivery in low- and middle-income countries is sometimes argued to be more efficient, accountable, and sustainable than public sector delivery. Public health care sectors are easy accessible to the community or people because of low cost of treatment ,mostly maintained by the government or the governing system of the country.low income for the people working in public health sectors, lack of people to run the sector,poor maintenance.low or zero inputs from the government to benefit poor people.Public sector services experienced more limited availability of equipment, medications, and trained healthcare workers.Public health care is usually provided by the government through national healthcare systems. Private healthcare sectors are run by non government, included unlicensed and uncertified providers such as drug shop owners, most patients appeared to access care in the private sector. Private health care can be provided through “for profit” hospitals and self-employed practitioners, and “not for profit” non-government providers, including faith-based organization. The government helps the private sectors by  increasing  the scope of private sector provision in health care as part of loan conditions to reduce government debt. However, critics of the private health sector believe that public healthcare provision is of most benefit to poor people and is the only way to achieve universal and equitable access to health care.

Solution to this problem can be:

by people fighting for their rights, the government being more keen towards helping people with their health.providing more financial options to help people, improving the quality of care, government implementation of quality care . Collaboration with high income countries to takeover the health care sectors with proper guidelines..providing equipment to smooth out work flow. Implementing and using technology for work. Online entry of data and other information to avoid errors and misplaced information.

3) yes value based care helps support the alignment of quality care benefiting the public health. It is about values and quality based rather than quantity. value-based care begins with the recognition that the workload must be share.
