Patient Profile: J.D. is a 50-year-old man who comes to the hospital with complaints of headaches and dizziness. He lives alone and works as a long haul truck driver. He eats mostly fast food on the road. He currently smokes ½ pack a day. Vitals: BP: 182/104, Pulse: 90, Temperature: 97.0, Respirations 24, Height: 5’10”, weight 248 lbs.

The present complaints of Mr J.D is headaches and dizziness. The occupation of the patient is long haul truck driver. He is a smoker use to take 1/2 pack of cigarette per day.

Vital signs on admission T- Normal, Respirations - 24, BP- 182/104 MM Hg.The baseline data reveal patient suffers with hypertensive crisis.The patient came with the complaints of headache and dizziness that is secondary to elevated blood pressure.

Hypertension : It is defined as the elevated systolic bp more than normal 120/80 mm Hg. Hypertension can be classified as prehypertension, mild,moderate and severe hypertension. The most common etiology for hypertension are stress, poor lifestyle practices like eating food rich in sodium, junk foods, secondary to cardiac diseases and renal disease. Most common symptoms of high blood pressure are head ache, blurred vision, fainting, palpitations.

SHORT TERM GOALS :

- Anti hypertensive drugs act as a first line of treatment in this case as patient Bp ranges from 180/104. On initiation of antihypertensives educate patient to low sodium to promote maximum effect of drug. Ask client to take drug on empty stomach one hour before or two hours after food.Periodical monitoring renal and hepatic function for early detection of complications.

- Stress reduction techniques : Stress plays a major role in hypertension. The recommended stress reduction techniques are active exercises, meditation, deep breathing techniques, biofeedback techniques, massage therapy, aroma therapy.

LONG TERM GOALS:

- DASH diet : Diet approaches to stop hypertension is a dietary approach to manage hypertension. The diet is rich in grains, vegetables, fruits, nuts, meat and dairy products. This helps to manage hypertension effectively.
