1) Evaluation and management codes are billed for the visit done by the patients to a provider in their office. A new patient is defined as the patient who doesn't receive any kind of consultation or care from the qualifies physician or other health care professionals belonging to same speciality or different speciality within three years of duration. The E&M code selected is 99204 for new patient office visit that meets a comprehensive history ( requires a chief complaint, four elements in history of present illness,and a MDM ,at least 10 review of organ systems).

2) Evaluation and management codes are billed for the visit done by the patients to a provider in their office.Established patient visit refers to he patient who receives any kind of consultation or care from the qualifies physician or other health care professionals belonging to same speciality or different speciality within three years of duration.The E&M code selected is 99213 as the documentation shows comprehensive history, expanded problem focuses exam ( that meets at least six organ systems examination), low complexity medical decision making ( two problem points and two data points).

3) Observation codes refers to E & M services to a patient admitted in observation unit and then got discharged on the same day. Observation might be initial and subsequent observe visit. Observation visits are short term treatments where the patient can be changed into inpatient or discharged based on his disease condition.
