C)The lack of development of protective immunity after vaccine series may be due to ineffective vaccines or immunodeficiency of the individual.

So the possibility of immunodeficiency should be checked in the individual.Tests should be done to identify any particular immunodeficiency disorders like agammmaglobuminemia etc

D) When a person is infected, initially IgM is secreted against the microorganism.It lasts for sometime then disappears.IgG is secreted after a few weeks but stays for life time .

So to confirm or reject suspicions of prior infections, serum IgG level should be checked.
