At the later stage or the end stage of the kidney failure the normal functioning of the kidney is critically altered along with the impaired kidney functioning resulting in disturbed GFR, which leads to the accumulation of the high concentration of the free drug as, the proper elimination of the drug does not happens. This results in an increased risk of drug toxicity.

Hence in this situation dose adjustment is highly recommended.
