Answer: Renal insufficiency is a condition where kidney doesn't work proper.

Nursing diagnosis : Excessive fluid retention or volume.

Desired outcome:

  • To improve the enhanced urine output.
  • Stable weight as well as vital signs
  • Reduce edema

Nursing intervention and rationale:

  • Monitor the urine gravity. ( In the case of renal failure, the specific gravity is usually equal to or less than 1.010)
  • Monitor the weight of the patient on daily basis. ( This will reveal about water retention as increase in 0.5 kg weight is also a significance of weight retention.
  • Assessment of edema specially on the legs.

Nursing diagnosis: Risk for reduced cardiac output.

Desired outcomes:

  • Manage the cardiac output
  • Normal the heart rate
  • Maintain healthy blood pressure.

Nursing intervention and rationale:

  • Monitor the blood pressure and heart rate. ( This will reveal the condition of the heart functioning.)
  • Assess the body color, nails. ( Pale colour indicates the presence of anemia.
