I have to do a nursing care plan. My patient had end stage renal diesease so my nursing diagnosis was Excess fluid volume. Can someone help me with short and long term goals for my patient for the nursing diagnosis and six interventions three for each... I am at a lost

Medical Diagnosis : ESRD (End stage renal failure).

NURSING DIAGNOSIS SHORT TERM GOALS LONG TERM GOALS NURSING INTERVENTION WITH RATIONALE
Excess fluid volume related to decreased GFR (Glomerular filtration rate) in ESRD. * At the end of todays shift able to effectively monitor the changes in fluid volume and treatment begin to control excess volume.

* After three days of nursing care make the patient able to maintain stable fluid volume and avoid reoccurance of increased fluid in the body to avoid complications like edema,breathlessness,heart diseases etc due to excess fluid in the body.

* Assess vital signs to know about changes in pulse and blood pressure due to excess fluid volume.

* Assess patient intake and output for diet planning based on current fluid status.

* Auscultate breath sounds to detect effect of fluid excess in the lungs.

* Give diuetics as per physicians order to reduce excess fluid volume.

* Change position immediately to avoid fluid retension.

* Restrict sodium intake to avoid fluid retension.

*Daily weight measurement to check fluid retension.
