I have to do a concept map my patient has dementia and renal failure ...I had to pick a psychosocial nursing diagnosis for him. So i picked accute confusion. can someone help me out with a short tem and long term goal for accute confusion. Also three interventions for the long and short term goals for a total of 6. Also if you can think of a better psychosocial nursinf diagnosis , I am all ears.

  • Dementia is a group of symptoms which is related to inability to catch a memory. Dementia interferes activity of daily living.
  • Maintain a proper environment to the patient, like proper lightning and ventilation
  • Orientation to time, place and day is importent. It includes Newspapers, familiar people, place, objects. It should be continuos and considered as long term goal.
  • Always handle the patient carefully and gently
  • Try to make the patient independent by encouraging him to participate in group activities
  • Maintain proper sleep pattern ( Minimum 8 hours). Better the patient can engage in some excercises during day time and maintain sleep in a particular time.
  • Encourage the patient to listen musics, movies, news or comedy series. Everything depends on patient likes and dislikes. It is like a distraction therapy
  • Encourage the patient to participate in social activities. Communication is the best source
  • Avoid repeated transfer from one place to other. It increases confusion.
  • Check, whether patient needs any sensory aids
  • Check for anxiety and manage if the patient shows any such symptoms
  • Encourage the patient to develop contacts with family members and friends who are familiar with him
  • It is adviced not to sedate the patient which lead more confusion
