I have to do a concept map i got my nursing dagnosis , i just need a 1.short term and 2.long term goal and 3 interventions for each. Pt is in a wheelchair so the nursing diagnosis was Impaired physical mobility. PLease help

Short term goal for impaired physical mobility-

. To avoid yhe hazards of immobility or to minimize potential for injury.

Intervention-.We have to do proper assessment of patient to be clear about disability.

We have ro check for functional level of mobility.

We have to provide safe and comfortable place to patient, with the use of safety and comfort devices.

Long term goal- To restore primary potential of patient.

Intervention- We have to provide rehabilitative care to patient

We have to educate patient and family members about dietary pattern, which will makes him better to ragain his primary potential.

We have to demonstrate some ROM(range of motion ) exercises which will be beneficial to patient in regaining his primary potential.
