• Skill: Impaired mobility
• Description of skill:Transferring a patient from bed to wheel chair
• Indications:
  • to enhance motor function by mobility
  • To ensure blood circulation
  • To relieve pressure
  • To prevent deep vein thrombosis in post OP patients especially in limb surgery, abdominal surgery.
  • Exposure to sunlight in bedridden patients
• Expected outcomes:
  • ​​​​​​​patient mobilized out of bed and a fresh atmosphere is provided to enhance healing and recovery
• Evaluation:
  • ​​​​​​​Patient able to cooperate during transfer if conscious
  • Active participation
  • Assess for any sore,restricted movement, limitation and range of motion
• Nursing interventions:
  • ​​​​​​​support the patient by holding the patient firm
  • Stand on the weaker side to provide additional support to the weaker limb or part
  • Transfer the patient as per the protocols and techniques
  • Encouraging patient to balance by self with support
  • Apply seat belt in the wheel chair after positioning on the wheelchair to prevent fall,leaning
  • Foot pad has to be adjusted and foot placed in the middle of it ,to prevent rotation of the limbs
  • Provide cushion support on the pressure points to relieve pressure while sitting
  • Place hands on the lap ,to prevent any accidental injury while moning from a room to external area
• Client Education:

  • Encourage to eat well because nutrition is necessary to provide energy

  • Prevent foot drop by applying splints
  • Provide back care,positioning every second hourly to relieve pressure
  • Maintain skin integrity in case of bed ridden patient
