Age: 27 years old Subjective information: Ms. Lyon comes to see you because she is worried about her weight and future health issues that may arise due to excess weight on her body. She reports having had weight issues since she was a child. Ms. Lyon states there is history of Type II Diabetes on her mother's side and history of hypertension and stroke on her father's side of the family, and she is worried about the connection between excess weight and chronic diseases. Ms. Lyon is not taking any medications. She is not a smoker and drinks alcoholic beverages only on weekends when going out. She reports no food allergies. Ms. Lyon reports really good appetite and enjoys eating a variety of foods. She states that due to her schedule she eats out 2-3x/week and the rest of the evenings she tries to cook, however, she says it is hard to include vegetables in her meals more than 2x/wk. She has a 9am-5pm-desk job and 1-2x/week! would meet with friends for dinner. She wakes up at 6am and goes to sleep at about midnight most nights of the week. On Saturdays and Sundays, she likes going for morning walks around the park for about 1-2 hours. Select - 24-hr recall: Breakfast: Sandwich on a broad roll with: 2 strips bacon, 1 fried egg and I slice American cheese, 1 Boz- cup coffee w/1/4c whole n d 2 packets of sugar. Lunch: Panini wchiel breast, roasted peppers, tomatoes, Swiss cheese on white pita bread. 12oz Pepsi, 1 Snickers bar. Mid-afternoon: 16oz hot café latte with whole milk and 2 packets of sugars. Dinner: 2 cups macaroni and cheese, 20oz Arizona half tea/half lemonade, le salad - lettuce, tomato, cucumber with regular Thousand Island dressing. Late-night snack: Lindt chocolate w/hazelnuts bar (-1/2 bar), le chamomile tea with 2 sugar packets. Ms. Lyon states she drinks water throughout the day - about 6-7 cups/day. Objective information: Ht: 63 in Wt: 178 lbs Blood pressure: 124/80 CHOL: 222 mg/dL. Laboratory data: LDL: 135 mg/dL TG: 160 mg/dL. HDL: 40 mg/dL FBG: 87 mg/dl.

1.The ,major health consequences which can be expected in this case is the very high risk for type 2 diabetes and cardiovascular diseases. This is because of her lifestyle, exercise, family history, alcohol ,sedentary work life,poor diet.
