Dosing errors at hospitals. Each year, approximately 1.3 million people in the United States suffer adverse drug effects (ADEs)—that is, unintended injuries caused by prescribed medication. A study in the Journal of the American Medical Association (July 5, 1995) identified the cause of 247 ADEs that occurred at two Boston hospitals. The researchers found that dosing errors (i.e., wrong dosage prescribed or dispensed) were the most common. The table summarizes the proximate cause of 95 ADEs that resulted from a dosing error.
