Answer #1

A nurse is obtaining a client's health history and discovers that the client takes loratadine, an over the counter drug.

The nurse should identify that

2. They can interact with other drugs. ( Over the counter drugs are the drugs for very common health problems which will available to consumers without prescriptions usually these drug does have major side effects but you should remember that these drugs can interact with other drugs and cause major side effects)

3. They should be included in the client's drug history assessment. (Its because OTC drugs are also the same drug as a prescription drug and they can interact with other drugs. so, it's very necessary to put it in medication history)

Answer - 2 and 3 options

Other options are wrong because of

1. They do not require the supervision of a nurse (every drug is required supervision of medical professionals)

4. They are less effective than prescription drugs. (They are the same drug)
