In the case of pneumothorax paramedics have difficulty ventilating because Air got away from the lung into the pleural space. In the long run, enough air gathered in the pleural space to make the mediastinum move towards the right. The collapsed left lung, increased intrapleural pressure, and rightward move make it hard to ventilate A.W . in much easier way to understand this is .paramedics couldn't ventilate A.W. since the pleural space is loaded with air, which at that point increase pressure on the lungs which doesn't enable the lungs to expand when air is breathed in
