Impaired Skin integrity r/t pressure ulcer secondary to poor circulation AEB disruption of epidermal and dermal tissue.

Please help me to create short term and long term goals. Goals must be SMART: Specific- Measurable- Attainable- Realistic- Time

#. Short term goals -

- maintain skin integrity

- avoid or reduce risk factors

- progressive would healing

- regain intact skin

- assess and treat existing wound

- prevention of pressure ulcer

- Patient will create a nutrition plan, in consultation with the nutritionist, within 1 week.

- Patient will meet with social worker within 1 week.

- Patient will consult with wound specialist within 72 hours.

- Patient will implement appropriate wound care procedures within 72 hours.

- Patient will show signs of wound healing, as evidenced by granulation tissue in the wound within 1 week.

- Patient will state strategies to promote wound healing following patient education.

#. Long term goals :-

- Display timely healing of skin lesions, wounds, pressure sores w/o complications

- Maintain optimal nutrition and well being

- Participe in prevention measures and treatment programs

- Verbalize increased self esteem, ability to manage situation.
