Present Problem: Ms. Sarah Jones is a 44 year old, white female who presented to her primary healthcare provider yesterday with complaints of recent episodes of shortness of breath that occur with minimal activity such as walking up a flight of stairs or with increased stress. Her symptoms are relieved with rest She denied any chest, arm or jaw pain but did have some diaphoresis and two episodes. She attributed her symptoms to her smoking one pack per day for th She was directly admitted to the telemetry unit from her physician's office. Currently, Mrs. Jones is complaining of tight" pain in mid chest region Personal/Social history: Ms. Jones drinks 3-4 caffeinated beverages per day and denies alcohol use. She has suffered anxiety since her mothers death. Her surgical history includes total abdominal hysterectomy 6 years ago and right carpa tunnel surgery 2 years ago. Her mother had a myocardial infarction (MI) at age 56 and died at age 66, secondary to complications of a coronary artery bypass graft. Her oldest sister died at age 54 from sudden cardiac death. Her father is living with a recent stroke and two younger brothers are alive and well. She fives and obesity, Her weight is 250 pounds and she is 5'5 tal with her husband and 3 healthy children, ages 14-18, all living at home. She is employed fuil time as a paralegal. 1. 1. Ms jone should checked for oxygen saturation and if it less than 90% then oxygen therapy should started. 2. as she develop stress during walking so advice for complete rest.and stabilisation of vitals. 3.as she develops addiction towards smoking. so because of direct admission to telemetry office she was unable to smoke and develop the withdrawal effect of smoking like profuse sweating or diaphoresis, nausea etc. it can be stabilise by sedatives. 4. she should advice for weight reducing exercises to avoid obesity related complications. 5. x-ray should done for mid chest pain. it is a warning sign of heart attack so required pathological test need to done. and the treatment should basrd on pathological report.
