A 12 year old boy has a headache, malaise, and poor appetite. He complains of back pain. His mother notices that his face is puffy, especially around the eyes. She becomes alarmed when her son passes smoky-colored urine and immediately brings him in the clinic.
Patient’s history reveals that ten days ago, he had a bad sore throat, fever, and upper respiratory infection and missed several days of school. Physical examination reveals an inflamed pharynx, elevated blood pressure (145/100 mm Hg), abdominal distention, and swollen ankles. His height is 4 feet, 8 inches (1.42m) and his weight is 91 lb (41 kg). Blood urea nitrogen (BUN) concentration is 30 mg/dl (normal 7-18 mg/dl) and plasma creatinine concentration is 2.0 mg/dl (normal 0.8-1.3 mg/dl). A 24-hour urine sample was collected and had a volume of 900 ml and contained 1.30 g creatinine and 0.90 g protein. Microscopic examination of the urine sediment revealed odd-shaped red cells, red cell casts, and leukocytes.
A tentative diagnosis of post-streptococcal glomerulonephritis was made and was later supported by the finding of antibodies against streptococcal exotoxins in the patient’s serum. Certain strains of streptococci provoke an immunological response that damages the kidney’s glomeruli. Treatment with penicillin, diuretic drug, a reduced salt intake, and rest at home were prescribed.
The urine output increased immediately after treatment, and the puffiness in the face and hypertension promptly disappeared. The urine color became normal in a few days, but the urine tested positive for red cells and protein for several weeks. Laboratory values indicated normal renal function one year after this episode.
The rationale of the treatment are
Usually if the nephroms once affected cannot be repaired but loss 9f kidney functions can be gained in weeks. In this patient it took nearly a year to completely recover (complete recovery means absence of RBC,protein ,casts in the urine)

A 15-year old boy complains of headache, dizziness, nausea, and feeling very weak. A 9-year-old boy has a history of a recent viral infection. He now feels faint and is feverish, and he is generally not well. He has to urinate frequently and is very thirsty. His breath smells fruity. He is seen in an urgent care clinic, where blood is drawn and urine collected for routine urinalysis. The following urinalysis results were obtained: Physical Appearance Color: Pale Transparency: Clear Chemical Screening pH: 5.0 Specific gravity (refractometer): 1.029 Specific gravity (reagent strip): 1.005...

1. A 27-year-old male comes to the clinic complaining of rash and puffiness around the eyes. On examination he is well built, blood pressure 146/96 mm Hg (normally 120/80 mm Hg), pulse 82 beats per minute. There is an erythematous malar (red) rash present on the face. Further investigation of his renal function shows serum creatinine 5 mg/mL (normal 0.7 -1.3 mg/mL) and blood urea nitrogen BUN 40 mg/mL (normal 5 - 20 mg/mL). Both creatinine and BUN are waste...

A 35-year-old man is admitted to the emergency department with chest pain; past history reveals other episodes of this same pain but of a shorter duration. Inquiry into his personal habits reveals that he is a cigarette smoker and that he follows a modified low-fat diet and engages in some regular exercise. His father died of ischemic heart disease at age 45, and other members of his family have had lipid-related disorders. Fasting blood is drawn for chemistry and hematology...
A 58-year-old male college professor sees his family physician because of increasing fatigue and weakness. He also reports pain in his lower back and arms when he walks. Physical examination reveals that the man has pale mucous membranes and hepatosplenomegaly. The physician orders a complete blood count (CBC) and urinalysis. A follow-up appointment is scheduled for the following week. Laboratory Data: The CBC reveals that the patient has anemia. His leukocyte count and differential count are normal, except for a...
