- Assessment of vital signs is the initial step in every patient which includes temperature, respiratory rate, heart rate , blood pressure,SPO2 and pain
- Catheterization of patient until the patient has control over the urinary bladder muscles.
- use of asepsis and hand hygiene while providing care and manipulating urinary drainage system .
- assess the patency of Foley's catheter .
- assess the colour , volume ,odour and components of urine .
- provision of perineal care after every urination and defecation.
- Teaching the patient kegal exercises to strengthen the perineal muscles.
- administration of IV fluids such as RL ,NS etc to avoid hypovolemia.
- monitor input and output .
- promote patient mobility and change position 2 hrly
- assess for pain and provide analgesics as prescribed.
- check for any kinks or obstruction in catheter and keep the urinary bag below bed level to allow continuous flow of urine due to gravity .
- Ensure that the urinary bag does not touch the floor .
