#. The main role of a surgical technologist in the cystoscopy room for a TURP is to set up the equipment prior to the procedure and ensure adequate fluids are available for inflow and suction are maintained for outflow.

#. TURP - Transurethral resection of prostate.

#. The procedure performed for :-

Confirmed diagnosis of BPH (benign prostatic hyperplasia). Symptoms include frequent urge to urinate, getting up at night to urinate, difficulty urinating, dribbling urine.

#. Positioning

Supine lithotomy with legs at 45 degrees, may switch to slight trendelenburg to help with viewing.

#. Draping and prepping

Prepping- suprapubic shave, betadine prep of perineal skin prep of penis, scrotum, and surrounding areas.

Draping- combination of laparatomy/perineal fenestated drape , including lithotomy triangular leggings.

#. Instruments

-perineal prep kit

-irrigation/kidney bowls

- isotonic solutions ( mannitol, sorbitol, glycine)

-Russian forceps

- 3 way Foley

- 10 cc syringe

- analgesic lubricant

- specimen containers

- y-type irrigation tube with clamp

-ellik evacuator

#. Procedure

-bladder and bladder neck assessed and landmarks confirmed

-sheath and obturator placed to drain bladder

-resectoscope inserted into urethra with irrigating fluid to clear vision for camera lens.

-left, right and median prostate lobes respected from bladder neck to verumontanum(elevation in floor of prostate portion of urethra where seminal ducts enter)

-vessels coagulation

-prostatic chips irrigated, flushed from bladder using ellik evacuator and collected

-cystoscopin inspection of bladder and neck, ensure all pieces collected, bleeding coagulated, sheath and orifices uninjured.

-retroscope removed, 3 way Foley placed

-Foley balloon inflated , continuous bladder irrigation applied.
