Community based nursing involves the small term and long term care of individuals and families to give strength to their capacity for selfcare. Care takes place in community locations such as home and clinic. The main focus is nursing care on individual or family.

Like other nurses, community health nurses can often find employment in healthcare facilities like local hospitals, trauma centers, and clinics. They can also be found working in health-related government agencies and non-profit organizations, as well as community health centers and research facilities.

The primary role of the public health nurse is to work within the community to support population health and deliver preventive health care services. Community dimensions of practice including health service planning, advocacy, and program effectiveness evaluation within and for the community.

The responsibilities of community health nurses are vast. They go to patients' homes, community centers, hospice facilities and to long-term care facilities. They also fulfill roles such as a home care nurse or visiting nurse, public health nurse, hospice nurses, school nurse, community advocate and parish nurse.
