INTRODUCTION
community health nurses play key roles in disease and injury prevention, disability alleviation and health promotion, as well as managing and providing care and follow-up across a broad range of settings.community health nurses offer education to community members about maintaining their health so that they can decrease the occurrence of diseases and deaths. They plan educational assemblies, hand -out fliers, conduct health screenings, dispense medications and administer immunizations.The community health nurses try to control or eliminate are infectious and sexually transmitted diseases, obesity, poor nutrition, substance abuse, smoking, teen pregnancy. Settings for community health nursing can be •(1) homes (2) ambulatory care settings (3) schools (4) occupational health settings (5) residential institutions, and (6) the community at large. Community health nursing practice is not limited to a specific area, but can be practiced anywhere.
COMMUNITY SETTING
Periyakulam is in theni district in the Madurai Region South Indian state of Tamil Nadu. As of 2011, the town had a population of 42,976. The utilization of community health services in community area of periyakulam are home visit, identify the risk cases , focus on the particular identified risk cases giving health education with the aim of prevent the diseases, identify the cases with disease and giving care with the aim of control the disease, identify the people needed for rehabilitative services and giving rehabilitative services for them,immunization, nutrition services especially for children, adolescence, pregnant and lactating women, encourage pregnant women for regular visit, deworming the children, family welfare services, treatment for minor ailments, geriatric services, prevention and control of communicable diseases, prevention and serve for outbreaks after disaster.
community health team includes community health nurse, teachers, stake holders, community leaders works collaboratively in home care for the promotion of health of people, prevention of diseases. The beneficiaries of community health services are especially,children, adolesence, pregnant women, lactating women, geriatrics, people with chronic diseases, care needy people.
COMMUNITY HEALTH NURSE ROLE IN HEALTH PROMOTION
1. Disease prevention
Community health nurses focus on long- and short-term care for disease prevention. Their work includes averting or controlling the spread of the flu and other communicable diseases. They work with patients to support diabetes self-management and improve diabetes control. They also work with mothers of newborns to reduce the rate of infant mortality and in schools to identify gaps in services.
2. Community educator
As educators, community health nurses focus on presenting materials in a clear and understandable format. They provide information to individuals, families, and communities that create a framework for healthy living and healthy choices. In schools they may teach sex education and HIV education classes. In the public, they hold classes and seminars on diabetes management. Overall, they focus on community health education as a step to preventive healthcare.
3. Leader
Community health nurses use evidence to implement policy changes and quality-based practices. They lead collaborative efforts to produce successful health outcomes and provide critical medical and social services in communities.
4. Researcher
As researchers, community health nurses collect and use evidence to execute positive changes for better health. Research is used to validate funding for public health programs, reduce inequalities in healthcare, and increase access to services.
5. Advocate
Public health nurses advocate on the local, state, and federal level to provide better access to healthcare, protect funding for public health programs, and reduce or eliminate health disparities. They help families arrange assistance through social services programs such as the Special Supplemental Nutrition Program for Women, Infants, and Children (WIC), which provides healthcare and nutritional services for low-income pregnant women, breastfeeding mothers, and children under age
6. Caregiver
Community nurses cross cultural, language, and literacy boundaries to shape the health and well-being of children and adults. They provide prenatal care and education for expectant mothers, including information about maternal nutrition, referrals for childbirth classes, and postpartum assistance. They also provide resources for parents to understand proper childhood development and discipline techniques.

Abstract
It is the responsibility of all nurses to incorporate health promotional and health educational activities into their professional roles. Much of the literature around this issue relates to the specific role of community-based nurses in the primary healthcare team, and identifies their unique position and responsibility for pioneering the universal acceptance and adoption of health-promoting practices. Community-based nurses are in a good position to commission health-related activities and integrate them into practice (Department of Health and Social Security, 1987; Department of Health, 1992, 1996). On the basis of this, one might expect that the results of studies in this area would identify evidence of good understanding, adoption, parity and support for such initiatives among community professionals. However, many studies have identified a state of confusion, poor understanding, a lack of parity between professional groups, a lack of professional support and training, and haphazard implementation of such strategies (Dines, 1994; Russell, 1997). Inconsistency in study findings is problematic and tends to present a skewed picture of current practice. It is suggested that community-based nurses may benefit from a change in the way they view the implementation of health promotional practices, and that policy issues related to practice could be further clarified and enforced.

The Association of Community Health Nursing Educators (ACHNE) provides a meeting ground for those committed to excellence in community and public health nursing education, research, and practice.
