A nurse is admitting a client who has a clostridium difficile infection. clostridium difficile is a bacteria that infects the bowel and can cause severe diarrhea. To prevent cross-infection of Clostridium difficile a nurse should take contact precaution measures.

Action a nurse should take is

1. Wear a gown and gloves when providing client care. (It's because Clostridium difficile can spread through direct contact with patients stools or fomite)

2. Wash hands with soap and water after contact with the client. (To prevent cross-infection)

Other options are wrong because

1. Use an N 95 respirator while providing client care. (There is no need of N 95 respirator because Clostridium difficile infection is not an airborne infection)

2. Assign the client a private room with positive airflow. (You may assign client with a private room with negative airflow because positive airflow is for immunocompromised patients it's not for infectious patients)

3. Ensure the client does not receive fresh fruit. (Fresh fruits are good during diarrhea so you should provide fresh fruit to clients)
