1.The statement " I will wear N95 respiratory mask when caring the client" indicates that the Licenced Nurse understand the teaching provided by the Nurse, as the mask is the most important thing that avoids 99.9% of infection transferring. Other options were not correct. Because too many visitors are not allowed and it's not necessary for the visitors to wear mask just for visiting.Also even though they are infectious they should not be separated or it's not necessary to be placed in a private room .Always a Glove should be removed first and then goggles, gown and other things should be removed, because contaminated gloves will further contaminate all other protective equipment.

2. It is necessary to secure the restraints with a square knot to avoid blocking of vessels which affects blood supply.

For the other options , always restraints should not be tied to side rails as this causes injuries, Its very important to remove restraints for every 2 hrs and 3 hrs is a long thing.At a time one remove 1 restraint is not correct because restraints should possibly removed as early as possible.
